North Jersey Health & Wellness (“we,” “us,” “the practice”) operates njhwllc.com. This policy explains what information the website collects, why, and what we do with it.
This policy covers the website only. It does not cover the medical records we keep about your care as a patient. Those are governed by our Notice of Privacy Practices and by federal and New Jersey law.
Information you give us
The website has three forms.
Our contact form asks for your name, email address, phone number, the office you’d prefer, and a message.
Our newsletter signup asks for your first name and email address.
Our Prompt Psych request form asks for your name, phone number, email address, preferred office, whether the appointment is for you or for a child or dependent, the patient’s age if they are a minor, and an optional note.
When you submit one of these forms, the information is emailed to our staff and stored in the website’s records.
Please don’t send detailed medical information through a website form. Email and web forms are not a secure or encrypted way to send health information. Tell us how to reach you and we’ll gather the clinical details by phone or at your appointment, where we can protect them properly.
Information collected automatically
Analytics. We use Google Analytics to understand how people find and use the site. It records pages viewed, approximate location based on IP address, device and browser type, how you arrived at the site, and certain actions: submitting a form, tapping a phone number, or clicking through to leave a review. We use this in aggregate to improve the site. We do not use it to identify individual visitors.
Search data. We use Google Search Console, which reports in aggregate which search terms bring people to the site. It does not identify individuals.
Spam filtering. Form submissions are screened for spam using Akismet, a service operated by Automattic. To do this, the contents of a submission are sent to Automattic’s servers for scoring.
Server logs. Our hosting provider keeps standard access logs, including IP addresses, for security and troubleshooting.
Fonts and icons. Some pages load fonts and icons from Google and Font Awesome. Those requests reach their servers and include your IP address.
Cookies
Google Analytics sets cookies to recognize returning visitors and measure sessions. Our hosting and caching software may set technical cookies needed for the site to function.
This site does not display a cookie consent banner and does not require you to accept cookies to use it. You can block or delete cookies through your browser settings. Blocking them will not prevent you from using the site.
Who we share information with
We do not sell your information. We do not share it with advertisers.
We share it with service providers who help us operate: our website host, our email delivery service, our email marketing platform, our spam filtering service, and Google for analytics. Each receives only what it needs to do its job.
We will also disclose information if the law requires it, or to protect someone’s safety.
How long we keep it
We keep website form submissions for 12 months, then delete them. If your inquiry became part of your patient record, that record is governed by our clinical retention policy rather than this one.
Google Analytics data is retained for 14 months.
Children
Our practice treats adolescents and children, and a parent or guardian may use our forms to request an appointment on a child’s behalf. We do not knowingly collect information directly from children under 13 through this website. If you believe a child has submitted information to us, contact us and we’ll remove it.
Your choices
You can unsubscribe from our emails using the link in any message we send.
You can ask us what information we hold about you from the website, ask us to correct it, or ask us to delete it. Contact us using the details below and we’ll respond.
Links to other sites
Our site links to Google Maps, our Google Business Profiles, and our social media pages. Once you follow one of those links, this policy no longer applies and theirs does.
Changes
We’ll post any changes to this policy on this page and update the effective date.
